Agreed - custom fields are a must for the contact form/widget. Only being able to intake name, email, and phone number is extremely limiting.

Specifically custom fields that map to their user profile, such as Member Number for our organization.

We would like to have opt-in options for types of email communication as well as address information to be used for snail mail and wealth screening.

This would be really useful, especially if a custom field could be mapped to a custom field for each user. I can see how this would be complex to implement in software, though, so even just allowing a form to have custom fields that are sent in a free-text "notes" field would be pretty good, too.
