Select multiple team members and credit donations to each participant in a single checkout.

User case: Ability to credit both a team and a team member during checkout. Currently, donors can only select either a team or a team member. If a team is selected, the donation is credited only to the team. If a team member is selected, the donation is credited to both the individual and their team. It would be helpful to allow donors to explicitly select both a team and a team member during checkout.
