Add custom fields to the Sign-up Form widget

The current Signup Form widget include name and contact information. Please include the option to add custom fields to these forms.

Enabling the sign-up form to be customizable, including the ability to add additional contact fields, such as address and phone number, as well as custom fields, would be highly beneficial for our non-profit.

Not only custom fields, but fields like notes and address. I would like to use this as the contact field for my website and it would need more fields for sure!
